Services Offered Under The Contract
Essential Services
- The management of patients who are ill or believe themselves to be ill, with conditions from which recovery is
generally expected, for the duration of that condition, including relevant health promotion advice and referral as
appropriate, reflecting patient choice where practicable.
- The general management of patients who are terminally ill.
- The management of chronic disease in the manner determined by the practice, in discussion with the patient.
Monitoring of coronary heart disease, diabetes, asthma and COPD is carried out by appointment with the practice
nurse.
Additional Services
- Cervical Screening
Cervical smears are recommended at least every three years for women below 65
years of age and who have not had a hysterectomy. The test is quick and painless
and can pick up very early changes in the cervix which are easily treated, and
which could otherwise eventually become malignant. A recall system operates in
this area so you should receive an invitation to attend if your smear is due.
When you require a smear please contact the surgery for an appointment with a practice nurse or a doctor.
- Contraceptive Services
Please make an appointment with a doctor for advice on family planning and all forms of contraception.
- Maternity Services - excluding intrapartum care
Please make an appointment with the midwife for antenatal care.
- Vaccinations and immunisations

Please make an appointment with the health visitor or a practice nurse.
- Child health surveillance
Please make an appointment with the health visitor.
- Minor surgery services
Cryocautery of warts and verrucae.
Directed Enhanced Services
- Improved access to primary care services
We aim to provide patients with access to a GP within 48 hours or with a
practice nurse within 24 hours.
- Childhood immunisations
By appointment with the practice nurse.
- Influenza immunisations for those 65 and over and other 'at risk' groups
Patients will be advised when clinics are organised.
- Pneumococcal vaccinations for those over 65
By appointment with the practice nurse.

- Holiday immunisations
If you are travelling abroad, especially outside Europe, you may need extra vaccinations. Please contact
the surgery at least eight weeks prior to travelling in order to complete a holiday travel form, as some
vaccinations need to be given a month or so before travelling. You may be required to collect a
prescription prior to attending for the vaccination with the practice nurse. Up-to-date information on
vaccinations required for different countries will be assessed prior to attendance at the clinic but is
also available from the Department of Health website or the Liverpool School of Tropical Medicine Travel Health Advice Line - (for pre-recorded country specific

- Practice nurse services
Available by appointment with the practice nurse or healthcare assistants.
Ear syringing, dressings, removal of stitches, blood tests, dietary advice, blood
pressure checks and new patient health checks.
- Postnatal care
By appointment with a doctor.
- Private medical examinations for insurance purposes
By appointment with a doctor.
Other Services
Health Promotion
We encourage all our patients to share the responsibility for their health, both
in preventing disease and in treating existing diseases. Prevention really is
better than cure. Many of the most serious diseases can be prevented by a healthy
lifestyle and without the need for drugs. Please feel free to discuss general health
and your lifestyle and ask for advice. Below are some of the ways that can help
you to live not only a longer, but also a fitter and more enjoyable, life.
Smoking
This is the single largest preventable cause of ill health in this country.
It is a major cause of cancer, heart attacks, angina and chest disease. If you
would like advice and help giving up please ask.
NONE OF THE DOCTORS IN THIS PRACTICE SMOKES.
Diet
A healthy diet not only helps control weight but also reduces cholesterol and
helps prevent heart attacks. Ask the nurse for advice.
Exercise
Regular exercise helps prevent heart disease as well as reduce weight and makes
you feel better. If you are overweight or out of shape please ask for advice
before starting vigorous exercise.
Blood Pressure
High blood pressure can, in the long term, increase the risk of heart attacks
and strokes. Reducing blood pressure can reduce these risks. Treatment does not
always require tablets. All adults are advised to have their blood pressure checked
at least every five years - if you have not had yours checked recently please
ask the nurse or doctor.
